Who is eligible?
Individuals 55 and over who are nursing home eligible.

A Holistic Healthcare Facility
Holistic healthcare means that the mind, spirit and body are all important to an individual’s health.
The PACE program offers its clients access to an on-site health clinic, where patients can see doctors and nurses to address their medical needs.
The site also features a dentist, mental health counselors and other healthcare professionals.

PACE accepts Medicaid/Medicare and private pay.
