- Must have been affected by a natural disaster such as- Tornado, Hurricane, Flood, Earthquake, Medical disease outbreak, drought (resulting in loss of income)

- Money, Checks for donations, DONATIONS ARE ONLY EXCEPTED WHEN WRITTEN OUT TO HOPELINE DISASTER RELIEF NEVER WRITE A CHECK OUT TO SOMEONE TELLING YOU OTHERWISE!

- Proof of disaster and address where you lived when it occured, ID for all adults, Social Security #'s will be needed if approved for service due to IRS reporting procedures, If any of these items were destroyed due to the disaster we are flexable

Notes
We can help you with anything you need in the event of a natural disaster no matter where you are located. We send a worker to meet with you go over the application and a needs form/ services form and you start getting the help that you need and deserve after going through a terrible disaster. Our email is open 24 hours a day, 365 days a year, and we currently have a new phone line being set up! 319-202-1987 is the current line plus the other will be up ASAP! Anything from clothing, toys, child care, job referal, home placement/finding we can help you with, ANYTHING at all we are there to help.
