About this facility

Mission
It is an ongoing challenge to establish and maintain adequate health services in rural Haiti. Prior to establishing our Borgne Health Alliance (Alyans Sante Borgne or ASB) in 2006, the H.O.P.E. clinic, was the only staffed clinic and laboratory or first-line professional medical care available to this community. Today ASB treats over 35,000 patients per year through mobile clinics and the community hospital at Fond la Grange. Despite significant progress since ASB’s startup, the lack of basic medicine, inoculations, vaccines and health awareness enables highly treatable and avoidable diseases, injuries and illnesses to become life-threatening and debilitating on a large scale. Our goal is to provide preventive and primary care medicine to the entire community. To date we have had significant success, but face ongoing challenges due to funding shortages to expand our work. ASB has developed numerous successful partnerships to channel global funds and foundation support into this community. * Alyans Sante Borgne/Borgne Health Alliance (ASB) – Since the formation of our partnership with MSPP (Ministry of Health) and the formation of ASB in 2006 we have grown to a staff of over 70 staff, including 4 physicians and a cadre of nurses, auxiliaries, technicians and community health agents. All staff are Haitian and many were born and raised in the community. H.O.P.E. is responsible for management of this health systems, which served more than 35,000 patients in 2009. We continue to fund this system providing staff salaries and much needed medicines and supplies.
* Basic Sanitation and Health Education – Develop and provide training and education in basic health. This is critical to reduce and eliminate the extremely high risks of disease due to improper sanitation, water and nutrition. HOPE’s Technology Center (Sant Teknoloji) has operated a water purification and sanitation program for the past five years. Staffed by local workers, this program offers vital first-line disease prevention facilities and education to the region.
* Village Outreach Service – This service provides traveling outreach programs to the remote areas of Borgne. Previously most of the population had never seen a health services professional. There are currently numerous staff making rotating visits to the community. More frequent visits with better equipped staff will eliminate many of the serious health risks early enough to greatly improve conditions.
* Infant and Maternal Health Program – One of Borgne’s most serious health risks is found in child-birth and post natal care. As an example, one of every eight children born in Haiti does not live to see the age of five. Many mothers die of post-partum complications. Our program provides health education and pre-natal/post-natal care with a goal to eliminate this very manageable cause of death and disease.

MSF came by yesterday to discuss what they can do to establish at least one ORC further and the went out to one of these sites with Dr. Voltaire. they promised to "do something" but we don't know what specifically.

Per my talk with Dr. Voltaire last night, we need disposable gowns for the staff, Aleze pads for patients beds, cots and more cots, and a tent!! we need to separate recovering patients who spend a day of recuperation before heading back to their homes. we had noticed some returning patients and identified that the weak ones or the ones who live far, don't have the strength and energy for the long trek home, on foot and in the sun. so the tent would be used for them.
we still have IV fluids and the ringers and ORS but would not mind having more on stock. we are still at 145/150 in patients so that stuff goes fast!

"...we had 5 return patients who had been discharged within the past 4 days (need to clarify the time of discharge to make sure) and this is a red flag for him (Dr. Voltaire). He attributes the relapse to a too quick discharge of certain patients who are sent home where they do not have the resources to keep recovering (food, water...), have too long a distance to walk back home and not strong enough for the haul, or were already weak and had other complications/infections and needed more time to get back on their feet.
I found this interesting and asked him to keep tab of these patients'
cases. He thinks that classic treatment modalities might not always apply to
situations like those found in rural Haiti."

11/25/10 Dr. Jim Wilson quoting Michael Sheilds of Project Hope
The HEAS previously shared emergency requests for assistance from the
community of Borgne and have been working furiously to mobilize
personnel and materiel to the community. However, it is still a
struggle. This is the most recent report from the community:

"I thank you for the yesterday’s huge efforts, the container (which
was finally released from port – not cholera specific but helpful in
materials), other materials and aqueous solutions on that flight from
PAP. It is too much to ask and I know that you are tired. I must
continue to push since you’ve never failed us in the past. The
recently expanded hangar is already filled, and the patients can do
nothing but cry out in fear and pain. We are going through [Lactate]
Ringers like water. Our completely inadequate level of staffing is
killing us from fatigue. Our current situation remains beyond our
abilities to control it; and without continued aid per our list of
needs, in the next 48 hours this health center could be more like a
cemetery. We need more personnel, more and larger tents, and
volunteers, meds, and sanitary supplies. It is truly an alarming
situation and without you Borgne is doomed. We thank you… "

11/23/10 LPar Note: Facility still in dire need for supplies and staff. HEAS group making attempts to respond to urgent needs. Shipment set to go got canceled. Group is still trying to respond.

Rosemarie Chierici reported that they have lost staff to higher paying MSF. I will alert CHHN and Konbit Sante of this "unexpected consequence" of MSF recruiting.

Coordinates for the helicopter landing spot (not a pad but used in the past by UN): 19.847598,-72.524679 (from Google Maps). It is just at the mouth of the Borgne River, adjacent to the main village.

The dirt airstrip is not safe to use for small planes.

UPDATE FROM Rosemarie Chierici/Michael Shields by phone 11/20/10:

Multiple Cholera Supplies needed. Per their director Michael Shields, they will contact MSPP for supplies and coordinate with the Cap Haitien Network. After supplies are secured within the region, we'll discuss potential solutions for any outstanding needs.

-Fatality rate at the clinic is 10-13%
-60 patients currently
-Low on Nursing Staff
-4 Doctors on site (1 on leave)
-Highest supply priority is IV Fluids/ORS/IV Supplies
-Catchment of area is 80,000 people
-Once the hospital supplies are secured and containment/treatment is improved, they will implement their mobile units to bring supplies/education to remote areas
-Currently, the patients are being housed in a makeshift hanger that existed for quake survivors and many are under tarps
-Large Military Tent needed
-Showers needed for disinfection
-Latrine installation is needed
-Water purification tablets are needed

She gave a list of needs which are posted. Urgently need large tent. They are set up for 20 and have 40 cholera patients. Also need personnel to go out into rural areas where sick people would not have transportation to the town.
