Gallatin Mayor Jo Ann Graves urged citizens to fill out a form on the city’s website with basic contact information to help report damages.

The form, which can be found by clicking on the "FEMA" logo at gallatinonthemove.com, only asks for basic contact information so that the city can email or mail the official damage report forms when they are received.

The damage detail form will come from the Sumner County Emergency Management Agency.

“It is very important we are able to provide FEMA with as much information on damages as we can. We certainly want Gallatin and Sumner County to be eligible for relief,” Graves said.

The contact form can be found by clicking on the “FEMA” logo at the city’s website www.gallatinonthemove.com. Residents without email or Internet can call the mayor's office at 451-5961 and ask for Pat Byars.

Special Projects Director Rosemary Bates said the city would "be happy to assist any and all" Sumner County residents.

Once the county receives a disaster designation from FEMA, individuals, families and businesses affected by flood damage can begin applying for federal assistance in the form of grants for temporary housing and home repairs, low-cost loans to cover uninsured property losses, and other programs.
